Preparing for Your Appointment
Your pediatrician wants you to feel confident and satisfied with every visit. When you come to your appointment, please bring the following information:
- All of your child's medications, vitamins and herbal supplements.
- Any forms that Mayo Clinic Health System requested.
- A list of questions or concerns for your pediatricians.
- A list of any recent life changes or major stresses.

- Early Childhood and Childcare Immunization
- Minnesota Sports Participation
- Wisconsin Sports Participation
After your appointment:
Soon after your visit, be sure to create a patient portal account for your child on our patient portal. You can see records and results, manage appointments, view instructions and review what the doctor told you.
Parents can create an account for their child until the age of 13. Once your child turns 13, child consent is required.
