Take the Community Health Needs Assessment Survey

Mayo Clinic Health System hospitals complete a community health needs assessment every three years. One part of this assessment is gathering community input through a survey. The results of this process help to create an updated community health improvement plan that guides the health system's community outreach for the next three years. Hospital surveys are on varied timelines.

Mayo Clinic Health System proudly works with hundreds of local partners to survey the community and execute implementation plans. These plans strengthen the quality of life, and health and well-being, in the communities Mayo Clinic Health System serves.

The guiding principles are:
  • Health is valued by all community members.
  • Health is determined by many factors, including social, behavioral, environmental and medical, and all of these factors are important.
  • Mayo Clinic Health System is committed to working with others to produce measurable improvements in the health of all community residents.
  • Strategies to improve community health are best determined and implemented through partnerships among health care providers, community members and community-based organizations.

About community health needs assessments

A community health needs assessment refers to a health assessment that identifies key health needs through systematic data collection and analysis. Federal regulations require that all hospitals in the U.S. study and report on the health needs of communities they serve.

About community health improvement plans

According to the Centers for Disease Control and Prevention, "A community health improvement plan is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process." Community Engagement staff use this plan to guide their community outreach work.
