    Advance Care Planning

    The best time to make an advance care plan is now.

    Take the time to document and share what matters the most to you about how you want to be cared for. Talk about your thoughts and wishes with your health care provider when you are in for routine checkups.

    To begin creating your advance care plan, follow these steps:

    1. Have conversations.
    2. Get more information from FAQs and Helpful Links, or call 715-464-7342.
    3. Create your advance directive.

    Regardless of your age or current state of health, if you are over 18, it’s important to create your plan, because the future is never certain, and anything can happen at any time.

    Advance directives can be in the form of a living will, designation of health care surrogate or other document naming an alternate decision-maker.
