    Community Health Needs Assessment in Fairmont

    Mayo Clinic Health System works collaboratively with local partners to regularly assess and address the health needs of its communities.

    In the Southwest Minnesota Region, we work with hundreds of community partners on collective efforts to strengthen the quality of life and improve the health and well-being of all community members.

    Community Health Needs Assessments identify priority issues that shape community engagement work.

    Community engagement work in the region aligns with these priorities:

    • Assessing social determinants of health
    • Improving mental health

    These principles guide our efforts:

    • Health is valued by all community members.
    • Health is determined by many factors, including social, behavioral, environmental, as well as medical, and all are important.
    • We are committed to working with others to produce measurable improvements in the health of all community members.
    • Strategies to improve community health are best determined and implemented through partnerships between health care providers, community members and community-based organizations.
