Community Health Workers

You may benefit from partnering with a community health worker if one or more of these statements apply:

  • I'm confused about how to care for my disease or condition.
  • I need help in navigating the health care system.
  • I'm confused about how to take my medications or what they do.
  • I need help scheduling healthcare appointments.
  • I don't have medical insurance.
  • I visit the Emergency Department a few times each month.
  • I have difficulty understanding my care team’s instructions (or following the care plan they give me) because of language, learning difficulties, or disability.

Community Health Workers can work with you to help address health goals, understand your disease or condition, advocate for your health, navigate the healthcare system, and figure out solutions and resources to address your concerns. Community Health Workers can meet with you at the clinic, discuss your concerns with you over the phone, or visit you in your home.

Health Equity Coordinators

Health Equity Coordinators assist patients with concerns like food, housing, transportation, and utilities. They look for resources in the patient’s community to help meet those needs and then connect the patient with those resources.

When you complete your screening questionnaire about concerns with food, housing, transportation, and utilities, a Health Equity Coordinator will automatically send you resources to your phone or email if you indicate you want help and how you would like to be contacted.
