If you're struggling to meet your basic health care needs or need assistance identifying community resources to help with daily living, a community health worker can help.
You may benefit from partnering with a community health worker if one or several of these statements apply to you:
- I'm confused about how to care for my chronic disease or condition.
- I need help in navigating the health care system.
- I'm confused about how to take my medications or what they do.
- I'm not able to make medical appointments due to transportation concerns.
- I don't have medical insurance.
- I visit the Emergency Department a few times each month.
- I've been admitted to the hospital recently.
- I have been diagnosed with a new condition.
Talk to your primary care provider for more information about enrolling in the Community Health Worker program. If you don't have a primary care provider, contact Mayo Clinic Health System at 1-877-412-7575.
