    How can a community health worker help?

    If you're struggling to meet your basic health care needs or need assistance identifying community resources to help with daily living, a community health worker can help.

    You may benefit from partnering with a community health worker if one or several of these statements apply to you:

    • I'm confused about how to care for my chronic disease or condition.
    • I need help in navigating the health care system.
    • I'm confused about how to take my medications or what they do.
    • I'm not able to make medical appointments due to transportation concerns.
    • I don't have medical insurance.
    • I visit the Emergency Department a few times each month.
    • I've been admitted to the hospital recently.
    • I have been diagnosed with a new condition.

    Talk to your primary care provider for more information about enrolling in the Community Health Worker program. If you don't have a primary care provider, contact Mayo Clinic Health System at 1-877-412-7575.
