How to participate
To submit a photo and reason for masking, complete this form. Submit your photo by 11: 59 p.m. on Dec. 4 to be considered for the project.
- Participants must be 18 years old or older.
- One person can be in the photo.
- The person pictured must be wearing a mask properly.
- The photo may not be taken in a direct patient care setting.
- The photo cannot be promotional in nature.

As everyone continues to live and work through COVID-19, the community is relaxing when it comes to preventing infection. That's a big reason why COVID-19 rates are surging across the Midwest.
And that's where your help is needed.
To combat this surge, Mayo Clinic Health System is working to spread the word about prevention practices such as wearing a mask, hand-washing and practicing social distancing. Everyone working together and practicing these prevention strategies will slow the spread of COVID-19.
And that will ease the surge and bring welcome relief to the community.
To get there, Mayo Clinic Health System needs your help. Mayo Clinic Health System is working on a campaign to remind the community about the many reasons why people should wear a mask. You can help by submitting a photo and a short sentence about why you choose to wear a mask. Selected submissions will be used in the campaign.

Your photo and quote will be used to produce individual posts that will be shared on Mayo Clinic Health System's social media channels. You are welcome to submit a photo of yourself at work properly wearing personal protective equipment and in work attire, or you can highlight yourself while masking at home or in the community.
