    Outpatient Home Safety Evaluations

    Occupational and physical therapists evaluate your home environment in order to assess your ability to return home safely. Therapists make recommendations on home health services that will be needed and coordinate with your family and caregivers to make your transition back home easier for the patients.

    Therapists will do the following to assure your living environment is set up to maximize your safety:

    • Evaluate cognitive and physical attributes
    • Practice transfers
    • Determine equipment (including the use of adaptive equipment and grab bars) and services that are needed to increase your safety.
    • Discuss any issues or concerns
    • Make recommendations and provide contact information for community services including home health care and home services
    • Determine if the patient is safe alone or if assistance/supervision is needed.
