    Care Teams

Primary care has traditionally been provided by a doctor and a nurse. That way of delivering care worked well for many years. But as healthcare has become more complex, we have discovered the benefits of including providers with different areas of expertise on our patients’ care teams.

Now, in addition to a doctor and a nurse, our teams also include nurse practitioners and physician assistants, registered nurses, licensed practical nurses, medical assistants, clinical assistants and other support staff. At the center of this team is you. We can also bring in a variety of specialty care providers like behavioral health experts, social workers and pharmacists.

The providers you see at an appointment will depend on the needs you have that day. Your team is committed to giving you exactly the care you need, in the most supportive and convenient ways possible.
