    Mayo Clinic Health System is offering sports screenings for athletes entering grades 7 to 12 in the 2022–2023 school year to fulfill Minnesota State High School League sports requirements.

    Student athletes are asked to be prepared for the screening and to:

    • Wear gym shorts and T-shirt.
    • Bring the completed Sports Qualifying Physical Exam Form from the Minnesota State High School League (MSHSL) website.
    • Have a parent or guardian accompany them to the screening, if under 18.

    This screening is not intended to take the place of ongoing care with your regular health care professional. Immunizations will not be offered. If your child has not had a well-child exam in the last two years or is unable to attend one of these events, contact your local clinic to schedule an exam in which they can be cleared for sports.

    If a health concern is detected during the screening, the provider will advise you to contact your primary care provider and will not sign the MSHSL physical form.

    Sherburn screenings take place on Friday, August 5, and Saturday, August 6. View the sports screening flyer for those times along with additional locations and dates.
