    Class Details

Mayo Clinic Health System is offering qualifying sports screenings for area school athletes entering grades 7–12 in the 2022–2023 school year.

    Student athletes are asked to:

    • Wear gym shorts and a T-shirt to the screening.
    • Bring the completed Sports Qualifying Physical Exam Form from the Minnesota State High School League (MSHSL) website.
    • Have a parent or guardian present if athlete is under age 18.

This screening is not meant to take the place of ongoing care with your regular health care professional. Immunizations will not be offered. If your child has not had a well-child exam in the last two years or is unable to attend one of these events, contact your local clinic to schedule an exam in which they can be cleared for sports.

If a health concern is detected during the screening, the provider will advise you to contact your primary care provider and will not sign the MSHSL physical exam form.

View the sports screening flyer for additional locations and dates.

Schedule a sports screening appointment online with Patient Online Services or the Mayo Clinic App, or by calling 507-594-2600.
